The Center at CenterPlace is Greeley's premier, dedicated short-term rehabilitation facility featuring 54 private suites, state of the art rehabilitation equipment, and care delivered in a hotel-like setting.

Our center is seeking team-oriented LPNs who are dedicated to patient outcomes and customer service.

The LPN is responsible for providing direct nursing care to patients. Such care must be delivered within the scope of practice and in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility.

Essential functions of the position:

• Delivers compassionate nursing care while achieving patient satisfaction and positive clinical outcome
• Administers and documents the administrations of medications and treatments per physician orders
• Assesses patients routinely and when changes of condition occurs. Communicates the information per company policy
• Maintains knowledge of federal, state, and local laws and regulations, as well as adheres to, upholds, and enforces all facility policies and procedures
• Provides information and instruction to patients and their families members as needed
• Ensures the patient care plan is implemented
• Communicates with nursing staff and care team effectively, both verbally and in writing and carries out directives

Must possess an active, unincumbered LPN license
